Cortisol and Your Adrenals

Two small glands, each about the size of a walnut, sit quietly on top of your kidneys. You never feel them working. Yet almost nothing in the body runs smoothly without them. They set the pace of your energy, hold the balance of your blood, steady your mood, and stand ready the instant life demands more of you. They are the adrenal glands, and most people never think of them until something goes wrong.

What the Adrenals Actually Make

The adrenals are not a single-purpose gland. They produce a small family of hormones, and this is exactly why so many systems depend on them:

The Daily Tide of Cortisol

In a healthy body, cortisol moves like a tide. It rises to its peak in the half hour after you wake — researchers call this the cortisol awakening response — giving you the lift to start the day. It then ebbs slowly through the afternoon and falls to its lowest around midnight, so the body can rest and repair. It was the physician Hans Selye, working at McGill University in the 1930s and after, who first mapped how the body meets pressure. He described a general adaptation syndrome — an alarm, then a stage of resistance, and finally, if the demand never lets up, exhaustion. He gave us the everyday word for all of it: stress.

When the Rhythm Breaks

Trouble comes from two directions. In one, the demand simply never stops — and the adrenals are held in a state of constant output. Cortisol runs high and flat, the tide loses its shape, and the wear of it accumulates. The neuroscientist Bruce McEwen called this slow accumulation allostatic load: the price the body pays for adapting to pressure that never releases.

In the other direction, the glands fall behind and produce too little — too little cortisol, too little aldosterone. This is the quieter, less-talked-about failure, and it is the one I know personally.

A personal word. I write this as someone who learned about the adrenals the hard way. My own body makes too little cortisol — a condition my doctors treat, and one I manage every single day with medication and close attention to my minerals. Because these same glands also make aldosterone, the hormone that holds the balance of sodium and potassium, my electrolytes slip out of line easily. Cortisol helps keep blood sugar steady between meals. When it runs low, blood sugar can dip too far — bringing deep fatigue, weakness, shakiness, and a stamina that gives out long before the day does. Appetite and weight often shift as well.

Fluids, Minerals & Blood Pressure

Through aldosterone, the adrenals decide how much sodium the body holds and how much potassium it lets go. When that signal weakens, sodium is lost, potassium can climb, and the body dehydrates — producing strong salt cravings, low blood pressure, and lightheadedness or dizziness on standing up.

Heart & Circulation

Cortisol is one of the body’s main brakes on inflammation. When its rhythm is disturbed, that brake works unevenly — leaving a person catching every illness that passes, healing slowly, and more prone to inflammatory flare-ups.

The gut is sensitive to adrenal hormones. Underactivity can bring nausea, loss of appetite, abdominal aching, and looser digestion, while the constant-stress pattern tends toward an unsettled, unreliable stomach.

Brain, Mood & Nerves

Cortisol crosses freely into the brain, so the mind feels these swings keenly — brain fog, trouble concentrating, a memory that slips, and moods that tip easily into anxiety, irritability, or low spirits. The stress circuit and the emotions are deeply intertwined.

Muscles & Joints

Weakness, aching, and cramping are common — partly the low energy, partly the mineral imbalance that leaves muscles poorly supplied.

Sleep & the Daily Clock

When the cortisol tide loses its shape, sleep loses its rhythm with it — hard to wake and get going in the morning, then wired but tired at night, and rarely truly restored by the sleep you do get.

Skin & Appearance

In one particular form of adrenal insufficiency, the skin darkens in patches as the body signals harder for the hormone it is missing — often one of the first visible clues a doctor looks for.

Resilience to Stress & Illness

Above all, weakened adrenals leave a person with little reserve. Cold, infection, injury, or a hard emotional blow — the very moments the adrenals are meant to carry you through — become far harder to weather. An Honest Word on “Adrenal Fatigue”

You will see the phrase adrenal fatigue used often online, usually to describe feeling tired and worn down. It is worth being clear and honest here. True adrenal insufficiency — the kind first described by the English physician Thomas Addison in 1855, and the kind I live with — is a real, diagnosable medical condition that a doctor confirms with blood tests and treats with medication. The looser popular idea of “adrenal fatigue” as a mild burnout of the glands is not an accepted medical diagnosis, and the research does not support it as a distinct disease.

That distinction matters, and not to discourage you. Chronic stress genuinely does disturb the cortisol rhythm — that part is well established — and caring for that rhythm is real, worthwhile work. But if you feel deeply unwell, the answer is not to guess and self-treat. It is to be properly checked, because the serious version is treatable once it is found.

How to Protect Your Adrenals

For most of us, the aim is simple: ease the everyday load on these glands and give them the raw materials they need. A few grounded, practical habits:

Do not let your blood sugar crash. Skipped meals and sugar spikes both pull on the adrenals to rescue your blood sugar. Steady meals built around protein, healthy fat, and fiber ask far less of them.

Consider gentle adaptogens — carefully. Certain traditional herbs are studied for helping the body handle stress: ashwagandha, rhodiola, and holy basil (tulsi). When to See a Doctor

These can point to true adrenal insufficiency, which is readily tested and very treatable once found. And one urgent note: a severe drop known as an adrenal crisis — sudden weakness, confusion, vomiting, severe low blood pressure, sometimes brought on by illness or injury — is a medical emergency that needs immediate care. Early awareness is what keeps a manageable condition from ever becoming a frightening one.
